The first woman that I saw today was 23years old. This was her first pregnancy and delivery. The baby was born around 2am this morning and I came in to see her around 9am. She delivered at 41 weeks to a baby boy born at 3.770 kg. She came in with contractions then received an epidural and pushed for about 20 minutes. She has a few stitches. She did some skin to skin and a little nursing in the delivery room. Around 6am baby received materna. She said she felt her breasts grow during pregnancy. I checked the baby and he looked good. She said there was no pain when he latched on. But she felt she didn’t really know what she was doing. I helped her latch the baby onto her breast. He was very sleepy but we managed to wake him and get him to nurse from her right side. Then mom showed me how she was able to get him to latch by herself. He sucked for a while and there was no pain. I had also checked her breasts myself and when I squeezed there was colostrum. When we tried switching sides for her to practice he spit up. I went over breastfeeding management and explained hunger signs, satiation signs, how many times per day, how long each feed, and went over the wet and dirty diaper amounts and colors. Around 1:30 pm she asked for me to come in again because her baby was making gagging sounds as if he would throw up. She asked me to pick him up and undress him to pass to her and as I did he spit up quite a bit of brown fluid. She was concerned but I explained it could be the amniotic fluid clearing out of his body. Then we tried waking him a bit more and she wanted me to help her nurse sitting in a chair. We tried the laid back position which was much more comfortable for her. I suggested that position because she didn’t look so comfortable in the cradle pose but also because of the gagging. Levana had come to check on me and took over and brought the baby back into cradle pose and helped her latch just fine. There was no real issue with the latch just mother wanted reassurance with the sleepy baby.
