Mom said that she has difficulty getting her baby to latch. She is always hysterically crying. We explained that she needs to count the 3 hours from the start of the feed not the end and that is probably why her baby is not latching because 4 hours have passed. When we arrived it was already2.5/3 hours so she was able to latch much easier. But we also explained to feed based on hunger cues and discussed hunger cues and satiation signs and just proper breastfeeding management. She also needed a bit of guidance what to do regarding how tippet chalk works and so we stayed to explain some basics. She was American and half of her pregnancy was monitored in Israel and half in America.
When her baby unlatched from the breast there was a stripe on the nipple that I assumed was from all the previous times of latching incorrectly. Grandma wanted me to explain to her what engorgement is and what a let down is and how to manage. I touched on the basics but explained that if she does proper feedings that it shouldn't get engorged and that a filling of the breast is not the same as engorgement. I gave her names of lactation consultants in Jerusalem area that speak English
